93. Hazard and Risk Analysis

Hazard and Risk Analysis shall be performed before authorization of consequential robotic work and updated when relevant conditions change.
The analysis shall consider the complete lifecycle:
- Transportation and site entry.
- Installation and setup.
- Calibration and testing.
- Normal execution.
- Human–robot collaboration.
- Pauses and temporary states.
- Maintenance and tool change.
- Fault recovery.
- Emergency response.
- Deconstruction and removal.
Hazards may include collision, impact, crushing, shearing, entanglement, dropped loads, structural instability, electrical exposure, utility release, fire, hazardous materials, cybersecurity compromise, incorrect identity, erroneous commands, loss of localization, and unauthorized human entry.
Risk evaluation shall consider severity, exposure, likelihood, detectability, ability to avoid harm, uncertainty, and the effectiveness of existing controls. Low historical frequency shall not justify ignoring a credible high-consequence failure.
Each identified risk shall be linked to:
- Preventive controls.
- Detection methods.
- Protective responses.
- Residual risk.
- Responsible authority.
- Verification evidence.
- Reassessment triggers.
The analysis shall examine combinations of failures and not merely single-component faults. Common dependencies—such as shared power, communications, maps, sensors, or software libraries—shall be identified.
Unresolved unacceptable risks shall block task authorization.
